Schedule an AppointmentUnderstanding Opioid Use Disorder and MAT
Opioid use disorder (OUD) is a treatable medical condition. A careful clinical evaluation is the first step toward understanding your situation and determining which treatment options may be appropriate for you.
Medication-assisted treatment (MAT) combines medication — when clinically appropriate — with ongoing support and monitoring. It is not a one-size-fits-all program, and it is not right for every situation. Your provider determines your plan individually.
At CJ Medical Center, Carlos Jimenez, DNP, PMHNP-C, ADHD-CCSP, FNP-C provides telehealth evaluation for adults with substance use concerns. What is available to you depends on your location, your clinical evaluation, and applicable requirements — an evaluation does not guarantee a prescription or a specific treatment.
Evaluation and Clinical Eligibility
A comprehensive evaluation helps determine whether medication-assisted treatment may be appropriate for you and what level of care you need. Situations that require supervised detoxification, intensive treatment, or in-person services may be referred to a higher level of care.
Your substance use history
What substances you use, for how long, prior attempts to cut down or stop, and any treatment you have tried before.
Your overall health
Medical conditions, mental health, current medications, and other factors that affect safe treatment planning.
Your goals and readiness
What you want from treatment, your concerns about starting, and what kind of support fits your situation.
The right level of care
Telehealth is not appropriate for every situation. Your provider will tell you honestly if your needs require services beyond what an online visit can offer.
Individualized Treatment Planning
Comprehensive evaluation
Your provider reviews your substance use history, health, current medications, and treatment goals through a secure video visit.
Discuss your options
Your provider explains which treatment pathways may be appropriate — including medication-assisted treatment where clinically appropriate and available for your location.
Safety review
Your provider discusses safety considerations and whether a higher level of care or additional evaluation is needed before treatment begins.
Agree on a plan
If care through this service is appropriate, you receive an individualized plan. An evaluation does not guarantee a prescription or a specific treatment.
Medication Options and Safety
Medication may be one component of treatment for opioid use disorder, but it is never automatic. Whether medication is part of your plan depends on your clinical evaluation, your location, and applicable federal and state requirements.
Buprenorphine/naloxone may be considered for initiation or continuation as part of individualized treatment for opioid use disorder when clinically appropriate and permitted by applicable federal and state requirements. A prescription is never guaranteed. Additional assessment, monitoring, testing, referral, or an in-person visit may be required.
- Individualized treatment recommendations based on your evaluation
- Medication options discussed when clinically appropriate — prescriptions are never guaranteed
- Ongoing support and monitoring within your care plan
- Coordination with counseling or recovery resources when needed
- Referral to a higher level of care when your needs exceed the scope of telehealth
- Follow-up visits to monitor your progress and adjust your plan over time
Monitoring, Follow-Up and Referrals
Medication-assisted treatment works best with regular follow-up. Your provider monitors your progress, reviews how you respond to your plan, and adjusts care over time. If your situation requires supervised detoxification, intensive outpatient or inpatient treatment, or services that cannot be delivered through telehealth, your provider will discuss appropriate referral options. For patients located in Florida, substance use-related concerns are addressed within the scope of our primary care services, and referral to external psychiatric providers may be recommended when clinically appropriate.

What is medication-assisted treatment (MAT)?
MAT combines medication — when clinically appropriate — with ongoing support, monitoring, and follow-up care for opioid use disorder. Whether medication is part of your plan is decided individually based on your evaluation and applicable requirements.
Will I receive a prescription for buprenorphine or another medication?
A prescription is never guaranteed. Buprenorphine/naloxone may be considered when clinically appropriate and permitted by applicable requirements. Additional assessment, testing, referral, or an in-person visit may be required before any medication is prescribed.
Can I start treatment remotely through telehealth?
CJ Medical Center offers initiation and continuation of medication-assisted treatment for eligible adults located in Nevada, Utah, Washington, and Arizona, when clinically appropriate and permitted by applicable federal and state requirements. Whether treatment can begin remotely and which requirements apply — such as additional assessment, testing, or an in-person visit — is determined during your individual evaluation.
I live in Florida. Can I use this service?
For patients located in Florida, substance use-related concerns are addressed within the scope of our primary care services — evaluation, clinical guidance, and care coordination — rather than a dedicated MAT program. Referral to external psychiatric providers may be recommended when clinically appropriate. Contact us to discuss the right visit type for your situation.
I live in California. Can I use this service?
No. CJ Medical Center does not currently provide telehealth services to patients located in California for any service, including Substance Use Disorder & MAT.
What if I need detoxification or a higher level of care?
Telehealth is not a substitute for supervised detoxification, emergency care, or intensive treatment. If your evaluation suggests a higher level of care, your provider will discuss appropriate referral options. In an emergency, call 911. For mental health crisis support, call or text 988.
How much does it cost?
The initial evaluation is $199 and follow-up visits are $79 where the service is available. Please confirm service availability for your location before purchasing a visit. Insurance is accepted in select states.
Do you treat children or adolescents?
No. CJ Medical Center provides services exclusively for adults 18 years of age and older.
In case of emergency: CJ Medical Center is not an emergency or crisis service. If you or someone you know is in crisis or experiencing a medical emergency, call or text 988 (Suicide & Crisis Lifeline), call 911, or go to your nearest emergency room.
