What Is a Comprehensive Psychiatric Assessment?

A comprehensive psychiatric assessment is a detailed clinical evaluation of your mental health — your symptoms, history, functioning, and goals — performed by a licensed psychiatric provider. It is designed for adults who want diagnostic clarity, a careful review of an existing diagnosis, or a structured starting point for mental health care.

You might consider an evaluation if you are experiencing new or worsening emotional or behavioral symptoms, if a previous diagnosis feels uncertain or incomplete, or if you have received differing opinions and want a careful, unhurried review.

At CJ Medical Center, Carlos Jimenez, DNP, PMHNP-C, ADHD-CCSP, FNP-C performs comprehensive psychiatric assessments for adults 18 and older through secure telehealth visits. The goal is accurate understanding — an evaluation does not guarantee a specific diagnosis or any particular treatment.

Conditions We Evaluate

A comprehensive assessment may be appropriate for adults with concerns such as the following. Some conditions require in-person testing or a level of care that telehealth cannot provide — your provider will let you know if that applies to you.

Depressive disorders

Persistent low mood, loss of interest or pleasure, changes in sleep or appetite, low energy, or difficulty concentrating.

Anxiety disorders

Excessive worry, panic symptoms, social anxiety, and related concerns affecting daily life.

Bipolar and mood disorders

Episodes of elevated or irritable mood, mood instability, and related symptoms that benefit from careful diagnostic review.

Schizophrenia

Evaluation of schizophrenia and related psychiatric concerns — including symptoms such as hallucinations, delusions, or disorganized thinking — with assessment of the appropriate level of care.

Adult ADHD

Attention, focus, impulsivity, and executive-function concerns in adults.

Trauma- and stressor-related concerns

Symptoms following traumatic or highly stressful experiences that affect mood, sleep, or functioning.

Unclear or overlapping symptoms

When symptoms are complex, previously undiagnosed, or do not fit neatly into one category, a comprehensive assessment helps clarify what may be going on.

What Happens During Your Appointment

Comprehensive history

Your provider reviews your current symptoms, psychiatric and medical history, prior diagnoses and treatments, current medications, and relevant family and social history.

Symptom and functioning review

You discuss how your symptoms affect your daily life, work, relationships, and overall functioning — with time to describe your concerns in your own words.

Safety review

A standard part of psychiatric evaluation includes reviewing safety-related concerns so that appropriate care and resources can be recommended when needed.

Understanding your results

Your provider explains their clinical impressions, discusses possible diagnoses when supported by your evaluation, and answers your questions. A specific diagnosis is not guaranteed.

Recommendations and next steps

You receive individualized recommendations, which may include treatment options, follow-up care, laboratory work, or referral to specialists or a higher level of care when appropriate.

Individualized Treatment Planning

Based on your assessment, your provider develops recommendations tailored to your specific situation. Depending on your evaluation, a plan may include some of the following. None of these apply to every patient — all recommendations are individualized, and prescriptions are never guaranteed:

  • Diagnostic clarification and documentation of clinical findings
  • Medication management when clinically appropriate and permitted by applicable regulations — prescriptions are not guaranteed
  • Behavioral and lifestyle guidance relevant to your mental health
  • Laboratory work or additional testing when clinically indicated
  • Referral to therapists, specialists, or a higher level of care when your needs exceed the scope of telehealth
  • Regular follow-up visits to monitor progress and adjust your plan
  • Coordination with other healthcare providers when needed

Follow-Up and Referrals

A psychiatric assessment is often the beginning of care, not the end. Follow-up visits allow your provider to monitor your progress, revisit clinical impressions as more information becomes available, and adjust your plan over time. When your evaluation identifies needs outside the scope of telehealth — such as specialized testing or a higher level of care — your provider will discuss appropriate referral options. If you are experiencing a mental health crisis or emergency, call or text 988 or go to your nearest emergency room; CJ Medical Center is not a crisis service.

Frequently Asked Questions

How is a comprehensive psychiatric assessment different from a regular visit?

A comprehensive assessment is a more thorough evaluation intended to clarify your diagnosis and guide treatment planning. It is designed for complex presentations, uncertain diagnoses, or adults seeking a detailed first evaluation.

Will I receive a diagnosis?

Your provider will share clinical impressions and discuss possible diagnoses when supported by your evaluation. A specific diagnosis is not guaranteed — some presentations require additional information, testing, or follow-up before a diagnosis can be made with confidence.

Do you evaluate schizophrenia?

Yes. Carlos Jimenez evaluates adults for schizophrenia and related psychiatric concerns, and develops individualized treatment recommendations. Because some situations require in-person care or a higher level of service than telehealth can provide, your assessment includes determining the appropriate level of care, and your provider will discuss referral options when needed.

Will I get a prescription at my first visit?

A prescription is never guaranteed. Medication may be recommended as part of an individualized plan when clinically appropriate and permitted by applicable regulations, based on your provider's evaluation of your needs.

Can I schedule an assessment if I already have a diagnosis?

Yes. A comprehensive assessment can serve as a diagnostic review or second evaluation when an existing diagnosis feels uncertain, incomplete, or no longer fits your experience.

How much does it cost?

The initial comprehensive psychiatric assessment is $279 with transparent self-pay pricing. Insurance is accepted in select states — please verify your coverage with us before scheduling.

Do you evaluate children or adolescents?

No. CJ Medical Center provides services exclusively for adults 18 years of age and older.

In case of emergency: CJ Medical Center is not an emergency or crisis service. If you or someone you know is in crisis or experiencing a medical emergency, call or text 988 (Suicide & Crisis Lifeline), call 911, or go to your nearest emergency room.
